Multipurpose Family Income Form

 

Every family needs to complete the Multipurpose Family Income Form! You can learn more about this form on SFUSD's website, where you can also watch a video about how to complete the form.

Create or login to your account

  1. Go to SchoolCafe.com. If you already have an account, login and skip to the "Apply" section below.
  2. Scroll down and select California from the list of states. For the school district field, begin typing "San Francisco" and select "SAN FRANCISCO USD" from the list.
  3. Click "Go to My District," then "Create a new account." Select "I'm a parent," then confirm the district.
  4. Enter your first name, last name, email address, and phone number.
  5. Create a username, password, and security question.
  6. Accept the terms and conditions and click "Create account."
  7. Click where it says "Click here to verify." Check your inbox for an email called "Please verify your email address." Find the verification code in that email, then enter it on the website and click "Verify."

Apply

  1. Click "Apply for Benefits," even if you will be refusing financial assistance.
  2. Select your preferred language, then view the household letter.
  3. Click "Save Applicant Information" to close the pop-up window. Check the box next to "I certify," then click "Next."
  4. If you don't already have a student listed, click "Add a Student." Enter your child's first name and last name, then answer the two yes/no questions. Click "Add this Student" then "Next."

Not interested in assistance...

  1. If you know that your household income exceeds the amount that qualifies for assistance, check the "Not Interested" box and click "Next."
  2. After you review your information, you have the option of completing some additional demographic questions.
  3. On the final page, check the box next to your name, then click "Sign" and "Submit My Application."

Interested in free or reduced benefits...

  1. If you believe that you qualify, click "Next," then answer the questions about SNAP, TANF, and FDPIR.
  2. Add all your household members and any income they receive, then click "Next."
  3. After you review your information, you have the option of completing some additional demographic questions.
  4. On the final page, answer the question about your SSN (social security number). If you have an SSN, enter the last four digits. Check the box next to your name, then click "Sign" and "Submit My Application."

If your child will be eating school lunches, you will also need an account at MyPaymentsPlus.com, where you will manage your payments and benefits.
